Very simply put.
a vocation i like the job some-one wants and chooses to do and learn, but because of or due to an accident or lack of skills in that area, or it is perhaps an older person returning to the workforce from many years of not working, they need help to be placed back or in that workplace/job.
That is what a Vocational Rehabilitation person is required to do. You may say to help them in their transition from one place, where they desire to do it, to where they can get that job!
Vocational Rehabilitation; is the man or woman for the above job description.

but if you previosuly did heavy labor--and now need to find a new less demanding job due to your helath--than they might help you with retraining
